Since 1975, Bridges Connecting Communities, (formerly Knox Community Volunteers), has been
dedicated to providing volunteer services to the City of Knox and parts of the Yarra Ranges.
Our services are available to elderly residents or those with a disability including their carer who reside in the City of Knox or the South West Yarra Ranges.
Bridges operates under the guidelines provided by the Department of Health and Human services through the HACC (Home and Community Care) policy.
Through this policy Bridges promotes the Active Service Model which incorporates an individual goal directed care plan for all clients.
Bridges Connecting Communities is supported by financial assistance from the Commonwealth and Victorian Governments.
